Acne-prone skin is not defined only by visible breakouts. Even when lesions appear localised, they may reflect a wider state of imbalance involving excess sebum, follicular congestion, microbial disruption, barrier stress and inflammatory activity across the skin and gut.
When this pattern persists, the skin may remain reactive between flare-ups. Congestion can recur, inflamed papules and pustules may appear, excess oiliness can become more difficult to regulate and post-inflammatory marks may remain visible after active lesions have settled.
In this case supervised by Dr Rosie Sutton, Founder and Director of Tweak East, the objective was not to dry out blemishes or treat each breakout in isolation. The priority was to support clearer skin by improving the biological conditions that influence congestion, inflammation and long-term skin stability.
Clinical focus

Acne is a complex inflammatory condition shaped by gut and skin microbiome disruption, immune imbalance, abnormal follicular keratinisation and increased sebum activity. Stress, hormonal fluctuations, high-glycaemic foods, occlusion and aggressive cleansing can amplify flare-ups.
Photographs were taken at T0 baseline and T20, week 20. The protocol was conducted under the supervision of Dr Rosie Sutton and combined daily intake of The Skin Biotic nutraceutical, one sachet daily, with twice-daily application of The Gentle Cleanser and The Active Cream.
The clinical priorities were:

How AWvi supported the skin
The Skin Biotic supported acne-prone skin from within by acting on microbial balance along the gut-skin axis and contributing to healthier inflammatory responses. In this case, its role was to support systemic regulation, barrier resilience and the internal conditions that influence sebaceous activity and cutaneous balance.
The Gentle Cleanser supported the topical phase by cleansing without stripping. For acne-prone skin, this is essential: excess sebum and pore-clogging debris need to be removed without weakening the barrier, disturbing the surface microbiome or triggering additional irritation.
The Active Cream helped reinforce the barrier with biomimetic lipid support while contributing to a calmer, more resilient skin environment. In acne-prone skin, barrier support matters because inflammation, harsh cleansing and recurrent lesions can compromise tolerance and slow recovery.

Visible evolution
After twenty weeks, the skin appeared clearer, calmer and less congested. Visible inflammatory activity was reduced, the skin surface looked more even and post-inflammatory marks appeared less dominant.
The change was not only a reduction in visible lesions. It reflected a more stable skin environment, with improved tolerance, reduced inflammatory expression and greater capacity to maintain balance over time.
This case illustrates a core principle of AWvi’s approach: acne-prone skin should not be managed only by attacking breakouts. When sebum activity, follicular congestion, microbial imbalance and inflammation interact, effective support must also protect the barrier, regulate reactivity and improve the conditions that influence recurrence.
